Rifamycins are bactericidal antibiotics that inhibit bacterial RNA synthesis by binding to the β-subunit of DNA-dependent RNA polymerase, thereby preventing transcription initiation. Members of this class include rifampin, rifabutin, and rifapentine, each with distinct pharmacokinetics tailored to tuberculosis, Mycobacterium avium complex, and prophylactic therapy for latent infections. Rifamycins exhibit potent activity against mycobacteria and select Gram-positive organisms. Their ability to penetrate intracellular compartments makes them essential for treating chronic and persistent infections. However, enzyme induction and rapid resistance development require combination therapy. Rifamycins remain foundational agents in global TB control strategies and other difficult-to-treat infections.
