Cephalosporins are β-lactam antibiotics classified into five generations based on their spectrum and resistance stability. They inhibit bacterial cell wall synthesis by binding to penicillin-binding proteins, providing bactericidal activity. First-generation agents exhibit strong Gram-positive coverage, while later generations expand toward Gram-negative organisms and β-lactamase–producing strains. Widely used in pneumonia, skin infections, urinary tract infections, sepsis, and surgical prophylaxis, cephalosporins remain among the most commonly prescribed antibiotics worldwide. Their favorable safety profile supports use in pediatrics and general medicine. Resistance mechanisms such as ESBL production and altered PBPs continue to drive development of cephalosporin–β-lactamase inhibitor combinations and next-generation cephalosporins.
