Macrolides

Macrolides are bacteriostatic or bactericidal APIs characterized by a macrocyclic lactone ring and known for their excellent activity against respiratory pathogens, atypical bacteria, and intracellular organisms. Agents such as erythromycin, azithromycin, and clarithromycin inhibit protein synthesis by binding to the 50S ribosomal subunit. They show strong tissue penetration, prolonged half-life, and anti-inflammatory effects that benefit respiratory diseases. Macrolides are widely used for pneumonia, bronchitis, pertussis, sexually transmitted infections, and Helicobacter pylori eradication. Their favorable oral absorption and diverse formulations support broad clinical use. Resistance mediated by efflux pumps or ribosomal methylation remains a challenge, driving development of ketolides and next-generation macrolides.
