Quinolones are synthetic broad-spectrum antibacterial agents that exert their activity by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication, transcription, and repair. Their excellent tissue penetration and oral bioavailability make them widely used for respiratory infections, urinary tract infections, gastrointestinal infections, bone and joint infections, and certain cases of sepsis. However, concerns such as tendinopathy, QT prolongation, CNS effects, and emerging resistance require careful prescribing. Despite these limitations, quinolones remain cornerstone therapeutics in many clinical settings due to their broad spectrum, rapid onset, and versatile dosing options.
