Polypeptide antibiotics are APIs composed of cyclic or linear peptide structures that disrupt bacterial cell membranes, leading to rapid bactericidal effects. They are active primarily against multidrug-resistant Gram-negative organisms by binding to lipopolysaccharides and altering membrane permeability. Due to nephrotoxicity and neurotoxicity concerns, systemic use is limited to severe infections such as carbapenem-resistant Acinetobacter or Pseudomonas. Topical and ophthalmic formulations are widely used for skin and eye infections. Polypeptides have regained importance in the era of antimicrobial resistance as last-resort therapies.
