Polyenes are antifungal antibiotics characterized by multiple conjugated double bonds that bind to ergosterol in fungal cell membranes, causing pore formation and cell death. Major agents include amphotericin B and nystatin, which exhibit broad-spectrum activity against yeasts, molds, and systemic fungal pathogens. Polyenes are indispensable for treating life-threatening infections such as cryptococcal meningitis and systemic candidiasis. While highly effective, traditional formulations of amphotericin B are associated with nephrotoxicity, leading to development of lipid-based delivery systems that improve safety and therapeutic index. Polyenes are formulated as injectables, topical creams, and oral suspensions, and remain central to antifungal pharmacotherapy.
